The Employee Contribution Template form allows employees to elect contribution amounts to be withheld from their pay checks during the year. Employees can specify an amount as an automatic deduction, or a lump sum, and the date to start the deductions.
After the Employee Contribution Template Form is submitted and processed, the contributions specified by the employee are made automatically on the specified date.
