Confidential Information Form

The Confidential Information form is used to record confidential information about the employee. Separate forms are shown for Canadian and US employees.

Submitted and processed forms update the corresponding fields in the employee’s records in People.

Note: The options available in the Disabled and Ethnicity drop-down lists can be viewed from HR Admin.
