Submit Safety or Health Incidents

To record employee safety and health incidents, you can submit a Safety & Health (USA) form.

Submitted and processed forms update the corresponding fields in the Employment > HR Incidents screen of People.

To submit a Safety & Health (USA) form:

  1. Go to People, open the employee profile, and click Forms.
  2. Click the Safety & Health (USA) form.
  3. Select an option in the Injury or Illness Type drop-down list.
  4. Enter the date in the Incident Date field.
  5. Enter the time the employee began work on the incident date in the Time Began Work field.
  6. Enter the time the event occurred on the incident date in the Time of Event field.
  7. If appropriate, select the injury and body part on which the event occurred in the corresponding drop-down lists.
  8. Select the location of your organization where the incident occurred in the Incident Location drop-down list.
  9. Select the Privacy Case checkbox if the identifying information of the injured or ill employee must be protected when the incident is reported. When the checkbox is selected, the employee's identifying information is replaced with the text “Privacy Case” on OSHA reports.
  10. Complete the following fields:
    • Where the Event Occurred
    • Task Being Performed
    • Object that Caused the Incident
    • Actions that Caused the Incident
  11. (Optional) If the incident resulted in a hospital visit, fill out the Hospital Information section, as follows:
    • Enter the name doctor the employee saw in the Doctor field.
    • If the employee visited an emergency room, or was admitted to the hospital overnight, select the corresponding checkboxes.
    • Enter the name of the hospital in the Hospital field.
    • Enter the street name and city in the corresponding fields. Select the state in the drop-down list and enter the ZIP Code.
  12. If the employee was unable to work, or unable to work at full capacity because of the incident, fill out the Time and Cost section, as follows:
    • Enter the date the employee returned to work in the Date Return To Work field.
    • Enter the number of days lost and the number of days restricted in the corresponding fields.
    • To track whether days were lost, select Yes, No, or Unknown in the Are Days Lost? drop-down list.
  13. Review and enter details in the Reporting Information section. Dayforce automatically populates the Date Incident Reported field with the current date, and the Report Filed By field with the name of the user who opened the form. If appropriate, select Yes, No, or Unknown in the Questionable Claim drop-down list.
  14. Enter the WCB Case Number in the field.
  15. Click Submit.
