Record a Change of Name or Marital Status

When an employee's name or marital status changes, you can update their records by submitting a Name and Marital Status form.

To submit a Name and Marital Status form:

  1. Go to People, open the employee profile, and click Forms.
  2. Click the Name and Marital Status form.
  3. Note: Fields that are already recorded in Dayforce are automatically populated.
  4. Change the fields as required under Name.
  5. Make any required changes to existing entries under Marital Status.
  6. To record a new marital status, click Add.
  7. Select the new marital status in the corresponding drop-down list.
  8. Enter a start date and, optionally, an end date.
  9. Attach additional information to the Supporting Documents section of the form as needed. See Attach Files to Forms.
  10. Click Submit.
