In Workflow Administration > Form Builder, the end-dated COVID-19 Check-In and COVID-19 - Report Work Location forms are shown under the System heading of the menu, enabling you to make copies and create custom COVID-19 forms, if necessary. You can copy the XML of the default forms and add items like disclaimers as needed for your organization. You can also create your own employee properties if there is more information to track.
