Record Health and Wellness Status

When an employee's tobacco use status changes, you can update their records by submitting a Health and Wellness form.

To submit a Health and Wellness form:

  1. Go to People, open the employee profile, and click Forms.
  2. Click the Health and Wellness form.
  3. Note: Any fields that are already recorded in Dayforce are automatically populated.
  4. Enter an effective start date.
  5. Select an option in theTobacco/Smoker Status drop-down list.
  6. Enter a date in the Date Last Used Tobacco/Smoked field.
  7. Attach additional information to the Supporting Documents section of the form as needed. See Attach Files to Forms.
  8. Click Submit.
