When an employee's tobacco use status changes, you can update their records by submitting a Health and Wellness form.
To submit a Health and Wellness form:
- Go to People, open the employee profile, and click Forms.
- Click the Health and Wellness form.
- Note: Any fields that are already recorded in Dayforce are automatically populated.
- Enter an effective start date.
- Select an option in theTobacco/Smoker Status drop-down list.
- Enter a date in the Date Last Used Tobacco/Smoked field.
- Attach additional information to the Supporting Documents section of the form as needed. See Attach Files to Forms.
- Click Submit.
