The Personal Information screen shows your name, date of birth, and address details. If your enrollment is configured to include tobacco use, the Do you use tobacco products? drop-down list is available. You must select Yes or No to update your tobacco use status. Dayforce creates a health and wellness record with the selected tobacco status, and the effective start date set to the enrollment event date.
