The Personal Information screen shows your name, date of birth and address details. If configured for your enrollment, in the Do you use tobacco products? drop-down list, you must select the Yes or No option to update your tobacco use status. Dayforce creates a health and wellness record with the selected tobacco status, and the effective start date as the event date of the enrollment.
