Benefits Forms

In the Forms screen (Forms tab in the original UI), you can fill out forms relating to your benefits, such as updating your address or marital status, or adding, editing, or removing dependents and beneficiaries.

The Available tab shows forms that are open for submission, and the Submitted tab shows all submitted forms. The Available and Submitted tabs aren’t available in the original UI.
