You can open the Communities feature by tapping Communities in the context menu of your Home screen. The Communities screen shows a list of all your communities, where you can tap a community to view or switch between them. When viewing a community, you can scroll through its latest posts and interact with them. The top of the screen shows the community name and profile image, so you always know which community you're in.
