Employees who work in Utah, where there is no official state tax form, can submit the Utah Copy of Federal W-4 tax form. This is a generic state tax form that is identical to the Federal W-4 form, except certain steps and instructional text that aren't relevant to state tax filing are blurred out so that employees cannot view or edit those areas:
Only the following lines in the form are editable:
- 1(c) for selecting filing status
- 4(c) for entering extra withholding
- The field at the end of step 4 for claiming exempt status
- Step 5 where employees enter their name to sign the form
When the form is submitted and processed, a record is added to the State Tax Info section of the Payroll > Tax Definitions screen in People.
