The EEO Sight Identification form is used to record a visual identification of the ethnicity of an employee for governmental record keeping and reporting purposes. The information collected isn’t used to identify any specific individual, and is used by the government for the administration of civil rights laws and regulations.
Employees can self-identify their ethnicity using the Confidential Information form. See Confidential Information Form.
The EEO-1 Filing report generates a CSV (comma-separated values) file compatible with the government’s EEO-1 online filing method.
