The following system forms are available for employees to enter information about COVID-19:
- COVID-19 Check-In: A voluntary form used by employees to report their status relating to COVID-19. If they’ve been tested, they might report the date and result:

- COVID-19 - Report Work Location: A voluntary form that employees can use to report their primary work location. This form assists in identifying individuals who might have come in contact with other employees in the event a COVID-19-related case is identified. Employees can select a location in the Value column and enter the effective dates:

When employees submit a COVID-19 Check-In or COVID-19 Report Work Location form, it’s processed in a workflow. The fields that are filled out are added as employee properties to the Employment > Employee Properties screen of their profile in People.
For example, the COVID-19 Check-in form includes the “Date of your test” question. When employees enter a date and submit the form, the COVID-19 Test Date employee property with the test date entered by the employee is added to the Employment > Employee Properties screen of their profile in People.

When added to an employee's profile by the form, these employee properties are assigned an effective date based on the date that the employee submitted the form. Only one active record for each employee property can exist on the employee's profile at a given time. For example, on March 30, 2020, an employee fills out the COVID-19 Check-In form, selecting Not Tested for the “Have you been tested for COVID-19?” question. As a result, Dayforce adds the employee property COVID-19 Test Results to the profile with an effective date of March 30, 2020 and a value of Not Tested. On April 2, 2020, the employee submits the form again, and selects Tested-Awaiting Results for this question. As a result, Dayforce end dates the previous COVID-19 Test Results property as of April 1, 2019 and adds a new record with an effective date of April 2, 2020 and a value of Tested-Awaiting Results. |
COVID-19 - Report Work Location form |
This form includes the Work Location field, which corresponds to the COVID-19 Location employee property. More than one active version of this employee property can exist at a time. For example, an employee might need to report working at more than one location in a single day. The effective date of this property is defined in the Effective Start field entered for the location in the COVID-19 - Report Work Location form.
